Change the Name in the Passport after Marriage

Documents Required:

• Duly filled Application (Click here for the Application for Alterations (Form O)
• Current passport
• Marriage Certificate (Please note that marriage certificates issued by a Japanese Authority should be authenticated by the Ministry of Foreign Affairs of Japan. A copy of a marriage certificate issued from Sri Lanka should be authenticated by the Ministry of Foreign Affairs of Sri Lanka).

Prescribed Fee: JPY 1,300

Change of Profession

Documents Required:

• Duly filled Application (Click here for Form O)
• Current passport
• Academic /professional certificate(s) with one photocopy each
• A letter from the employer confirming the profession

Prescribed Fee: JPY 1,300

Other Amendments

Documents Required:

• Letter of request
• Current passport
• Duly filled Form (Click here for Form O)
• Passport

Prescribed Fee: JPY 1,300

Please note that all documents issued in Japanese should be issued submitted with a certified English translation. If the return of the documents is desired by mail, additional amount of JPY 800 to be paid along with self-addressed envelope or stamped Self-addressed envelope to be sent

How to Submit the Application

You can submit the application to the consular division of the Embassy by post or by hand. The Consular Division is open from Monday to Friday (Acceptance and issuance of documents between 09:15 - 13:00 and 14:00 - 16:00) The physical and mailing address is as follows,
