Required documents;

Renewal of Driving Licence :

• Duly filled MTA30/2 Offline application (Issued by the Embassy). Please complete the application using a black colour pen. Fingerprint and signature has to be placed in front of the authorized officer at the Sri Lanka Embassy.
• Current Driving Licence issued by Department of Motor Traffic Sri Lanka with a photocopy
• Medical Certificate (from respective country) with the test results of (i) eye sight, (ii) hearing, (iii) height and (iv) weight. (Documents issued by Japanese Language should be submitted with an authorised English translation)
• Valid Passport and copy of the data page of the Passport
• National Identity Card (N.IC.) (it’s not mandatory if the N.I.C. number is mentioned in the passport)
• Two black and white passport size photographs (3.5 c.m. x 4.5 c.m.)
• Self addressed envelope
• Service fee of USD 115
• Penalty fee for each year after the expiration date will be USD 10
• Processing fee of JPY 2000

Duplicate of Driving License :

• Duly filled MTA30/2 Offline application (Issued by the Embassy). Please complete the application using a black colour pen. Fingerprint and signature has to be placed in front of the authorized officer at the Sri Lanka Embassy.
• Current Driving Licence issued by Department of Motor Traffic Sri Lanka with a photocopy
• In lieu of the loss of driving license, complaint report made to the police station in Japan. (Documents issued in Japanese Language should be submitted with an authorised English translation).
• Valid Passport and copy of the data page of the Passport
• National Identity Card (N.IC.) (it’s not mandatory if the N.I.C. number is mentioned in the passport)
• It is mandatory to mention the Driving Licence number or N.I.C. number to trace back details in the computer system
• Two black and white passport size photographs (3.5 c.m. x 4.5 c.m.)
• Self addressed envelope
• Service fee of USD 115

• Processing fee of JPY 2000

Change of personal details of a Driving License :

• Duly filled MTA30/2 Offline application (Issued by the Embassy). Please complete the application using a black colour pen. Fingerprint and signature has to be placed in front of the authorized officer at the Sri Lanka Embassy.
• Current Driving Licence issued by Department of Motor Traffic, Sri Lanka
• Valid Passport and copy of the data page of the Passport
• National Identity Card (N.IC.) (it’s not mandatory if the N.I.C. number is mentioned in the passport)
• Evidence documents related to the change made, i.e. Birth Certificate, Marriage Certificate, Divorce Certificate etc. for the name change and resident certificate for address change.
• Two black and white passport size photographs (3.5 c.m. x 4.5 c.m.)
• Self addressed envelope
• Service fee of USD 115
• Penalty fee for each year after the expiration date will be USD 10
• Processing fee of JPY 2000

Due to following Discrepancies your document might get rejected at the point of scanning by the Department of Motor Traffic, Colombo - Sri Lanka.

• Incomplete application/lack of supporting documents
• Using photocopied application
• Application filled by using other than black colour pen (use only black colour pen)
• Anything drafted out of scanning area
  
• Coloured/incorrect size photograph (Black & white photograph only 35 mm x 45 mm)
• Finger print kept by using general purpose inkpad

How to Submit the Application

The applicant should come in person to the Consular Division of the Embassy. The Consular Division is open from Monday to Friday (Acceptance and issuance of documents between 09:15 - 13:00 and 14:00 - 16:00). The address is as follows:
