REGISTRATION OF BIRTH

The Embassy of the Democratic Socialist Republic of Sri Lanka

An application for registration of a birth must be made within 03 months of the birth. If the application is made after 03 months, the reason for the delay should be given in writing when making the application. Only the Sri Lankan father or mother of the child can submit and application for registration of birth of a child.

Please submit the following documents to the Embassy in this regard:

  • Duly completed application Form (please complete in English)
    If the birth was less than 03 months ago ‘Form B4’ Declaration of Birth under Section 16 OR
    If the birth was more than 03 months ago ‘Form B6’ Declaration of Birth under Section 24

  • Duly completed application form 0I under Citizenship Act Application forms for Registration of Births Citizenship 1 Form) and 02 photocopies.
  • Certificate of Birth Report "Shusshou Todoke Kisai Jiko Shomeisho" issued by the Japanese city office that registered your child’s birth and 02 photocopies.
  • The above Japanese Certificate of Birth Report "Shusshou Todoke Kisai Jiko Shomeisho" should be translated into English by an authorized translator and 02 photocopies of the translation.
  • Following documents of the parents:
    • Passports and 02 photocopies each of the data page of the passports.
    • 02 photocopies each of the ‘observations /alterations’ page of the passport if there are any observations/alterations.
    • 02 photocopies each of the Resident Cards.
  • Original marriage certificate of the parents and 02 photocopies (certified English translations will not be accepted).
    If the original marriage Certificate is in Japanese it should be translated into English by an authorized translator and 02 photocopies of the translation. If the marriage certificate is issued in a language other than Japanese, it should be certified by the relevant Embassy in Japan.
  • Original birth certificates of the parents and 02 photocopies. (certified English translations will not be accepted)

    If one of the parents is Japanese, in lieu of his/her birth certificate, the original family registration issued by the City Office should be submitted together with 02 photocopies. If a foreigner, the birth certificate should be certified by the relevant Embassy in Japan. The English translation of the above document should be done by an authorized translator and submitted with 02 photocopies of the translation.
  • Two envelopes with your address in Japan written.
  • Prescribed Fee for a birth registration: JPY 5,000 for birth registration, JPY 4000 for citizenship registration and JPY 1600 for postage (Total JPY 10,600)

    In addition, for registration of birth for a child more than one year old, a fine of JPY 1500 will be added for each delayed year. In such case the birth certificate is issued under the approval of the Registrar General/Colombo and will take 08 weeks to issue the birth certificate.
    Incomplete applications will not be accepted by the Embassy and will be returned back.

    Consular fees for Registration of birth

    a. Registration of birth        – JPY 5,000

    b. Citizenship certificate     – JPY 4,000

    c. Postage fee                      – JPY 1,600

                    Total                      – JPY 10,600

    d. Surcharge for applications submitted after one year of birth of the child – JPY 1,500 per year.

How to Submit the Application

You may submit the application to the Consular Division of the Embassy by post or by hand. The Consular Division is open from Monday to Friday (Acceptance and issuance of documents between 09:15 - 13:00 and 14:00 - 16:00). The address is as follows:

Notes:

* This Embassy is not responsible for any document lost in the post.

* Submission of forged documents and making false declarations relating to citizenship is a punishable offence.

* Fees will not be refunded in these circumstances.

* Incomplete applications will be returned and consular fees will not be refunded.

* For application forms duly completed the Embassy will take 04 weeks to issue the birth certificate. Citizenship certificate is issued by the Department of Immigration and Emigration in Colombo and will take around 06 months.
