REGISTRATION OF DEATH

Application Instructions

  • Declaration of Death (Declaration of Death under Section 30) has to be made by a close relative or friend or any other person who was with the deceased at the time of death.
  • The following documents are required for registering the death.
    • The Japanese Death Certificate "Shibo Todoke Kisai Jiko Shomeisho" issued by the Japanese city office where the death was registered.
    • Passport and other available identification documents of the deceased to prove Sri Lankan citizenship.
    • Police report
    • Original Marriage Certificate of the deceased (if married)
    • Original Birth Certificate of the deceased
  • The Embassy charges a fee of JPY 3000 for the registration of a death. Fees should be paid by cash.

Repatriation of Human Remains to Sri Lanka

If the deceased person was a Sri Lankan National following documents should be certified from the Ministry of the Foreign Affairs of Japan and legalized from Sri Lanka Embassy in Japan

  • The Japanese Death Certificate "Shibo Todoke Kisai Jiko Shomeisho"
  • No Infection Certificate
  • Embalming Certificate
  • Original current Sri Lankan passport (Foreign Affairs of Japan certification is not applicable)
  • Flight Details (Foreign Affairs of Japan certification is not applicable)

Cremated Remains (Ashes):

If you wish to take cremated remains to Sri-Lanka, please ensure you have the following documents.
Local death certificate
Cremation certificate
Please note: Death & Cremation Certificates should be certified by the Ministry of the Foreign Affairs of Japan and legalized from Sri Lanka Embassy in Japan.
Further to this, you should check with the airline and airport(s), which you will be travelling through, for their requirements.

* This Embassy is not responsible for any document lost in the post
* Submission of forged documents and making false declarations relating to citizenship is a punishable offence. Fees will not be refunded in these circumstances.
* Incomplete applications will be returned and consular fees will not be refunded.
