Documents required

• Life / Residence Certificate (Download)
• Original Passport with a photocopy (A4 size)
• Sri Lanka National Identity Card (NIC) with a photocopy (A4 size)
• Pension Card with a photocopy (A4 size)

Procedure

Applicant must visit in person to this Embassy with the above mentioned documents to attest the Life/ Residence Certificate.
A scanned copy of the attested certificate will be emailed to the Department of Pensions by the Embassy.

Service Fee

This document is issued free of charge.

Please use the following email address when sending Life/ Residence Certificates to the Department of Pension through email after doing necessary attestation by the competent authority of the Sri Lanka Embassy in your current country of residence.

If the applicant intends to submit the attested certificate by himself/ herself, Department of Pensions requests the applicants to refrain sending the Life/ Residence Certificates by Parcel Post as it costs extra for the clearances from Colombo.
